  5. Review Dental Record

  6. UKCD DENTAL CHART • Chronologic record of pertinent information • All forms with a number/letter are placed in the chart in order – number first, and then letter • All forms without a number plus radiographs go in the front pocket

  7. DENTAL RECORD CON’T • Treatment plan is attached to the front inside cover • The radiographic record(1B) is attached to the inside back cover • All progress notes go to the back of the chart in chronological order

  8. Information on the front inside cover • Student assign. • Missed appointments

  9. Organize Matierials for Appt. • Sign up for appropriate tray with sterilization • Collect forms needed not already in chart • Gather things to assist with data collection(bp apparatus, red/blue pencils,fan, etc) • Note day and time of appointment

  10. DAY OF INITIAL APPOINTMENT Check cubicle assignment

  11. ORAL DIAGNOSIS I(ODI) • Get tray from sterilization • Set up tray and any materials needed(gauze, bp cuff, patient napkin,etc.) • Retrieve patient chart from basket in rear of clinic • Get necessary forms

  12. FORMS NEEDED FOR ODI • Medical History(will already be in chart if patient went through preadmiss) or Axium • Dental History (Axium/electronic) • Clinical Eval (4A) • 1D • Progress Note(may or may not be in chart/axium)

  13. MEET PATIENT IN WAITING AREA • Meet patient and introduce yourself • Seat your patient and explain the nature of the appointment

  14. BEGIN DATA COLLECTION!! • Confirm with supervising faculty your planned activities(OD or team leader) • Review medical history • Dental history • Soft tissue exam • 1D

  15. MEDICAL HISTORY(3A) • This form is electronic • Review all systems and follow up on any positive responses • Review all medications • Obtain vital signs(bp and pulse) • Assign an ASA

  16. MEDICAL HISTORY CON’T • Identify systemic diseases that might need a medical problem worksheet • Identify things that might affect the routine way dental treatment is delivered(red tag on chart)

  17. MEDICAL HISTORY CON’T • Identify medications that would require a prescription worksheet • Sign the form

  18. DENTAL HISTORY(3B) • This form is electronic • You and the patient complete form • Review the form(front and back) and follow up on positive comments • You and the patient sign the form

  19. CLINICAL EXAMINATION(4A) • Perform a thorough soft tissue exam

  20. Faculty will indicate additional data appropriate for patient Radiographs

  21. OTHER DATA MAY INCLUDE • Consultations • Impressions • Vitality testing • Preliminary treatment • ETC.

  22. Completion of ODI • Inform attending faculty of your plan • Allow time to reappoint patient, complete paper work, etc before end of clinic(12 or 5) • Reappoint patient with coordinator • Progress note • Check out at computer

  23. PROGRESS NOTE • Date • SHAPED format • Signed by student and faculty
