StarPanel 3 1 2 • Click on: • Must log in to StarPanel to complete registration forms after Patient has been admitted in Medipac by Admitting • Select patient • Select Enter Data • Select Forms • Forms (from yellow tab) • Search Title Admitting Forms • Check Mark Admitting forms (English & Spanish) • Select radio button identifying entry point (Inpatient for scheduled cases & ER for patients coming for Triage)

  4. Completing Consents Select Inpatient or ER(for Triage) ADD Row is only used if Consent can not be obtained • Red Tab Indicates Form to be completed • For L&D and 4SMC • Consent to Treat • Payment • Whiteboard Consent • HIPPA

Completing Consents • Steps to Obtaining Signature • To allow patient to view the consent form on the signature pad while signing • Click and hold control key • Click on “Click to Sign” • Until the consent has been signed the “Click to Sign” box will be red • Scroll to top and choose next form

  6. Consent for Treatment • Must obtain signature upon admitting process • Must use appropriate script to explain forms • “By signing this form you agree to let us treat your child. You have the right to refuse any tests or treatment you don’t want when your doctor (health care provider) talks with you.

Consent for Treatment Form Red Tab Indicates Form to be completed “Verbal Consent by Whom” when necessary Click and Hold Control Key “Click to Sign for signature.” When “Click to Sign” is no longer redscroll to the top choose next form Copies of all consent forms can be printed upon request Click to Print Hard Copy of Patient Rights and Responsibilities Upon Request

  8. Payment • Must obtain signature upon update process • Must use appropriate script to explain form • “By signing this form, you agree to let us bill your insurance. You also agree to pay for anything not covered by your insurance.”

  9. Payment Form Government Payer Certification must be completed “Click to Sign for signature. When “Click to Sign” is not longer redscroll to the top choose next form Copies of all consent forms can be printed upon request

  10. Whiteboard(Allows name to be displayed) • Must obtain signature upon registration process. • Must use appropriate script for explaining the form to patients/legal representative • “By signing this form you acknowledge that your allowing your name to be displayed on the hospital whiteboard.”

  11. Whiteboard Consent Click and Hold Control Key “Click to Sign” for signature. When “Click to Sign” is not longer redscroll to the top choose next form

HIPAA(Notice of Privacy Practices) • Must obtain signature upon registration process. • HIPAA forms will be completed once a year • Must use appropriate script for explaining the form to patients/legal representative • “By signing this form you acknowledge that you’ve been offered a copy of our Privacy Practices.”

  13. HIPAA form(Notice of Privacy Practices) Click and Hold Control Key “Click to Sign” for signature. When “Click to Sign” is not longer redscroll to the top of forms to finalize Click to Print Hard Copy of Notice of Privacy Practices Upon Request

  14. Finalizing Forms To finalize/save the forms, click the red ‘Save as Final’ button. ****DO NOT SAVE AS DRAFT**** The yellow buttons are specific to each form and cannot be used for finalizing the forms. There’s no need to use this ‘Save’ button.

  15. For signed copy, click on the forms from the Patient’s Chart and print forms using the Yellow actions box

  16. Saved Forms Under Wrong MRN? • If you make a mistake with saving forms under the wrong MRN, save multiple sets of forms for the same patient, or have other issues that need to be corrected, notify the HELPDESK @ 3-HELP of the error. • Provide the following information: • incorrect MRN and correct MRN it should be moved to • which forms were completed • 2. Which forms were completed • 3. Date it occurred Medical Information Services Mgmt must make the corrections

  17. What To Do If Patient/family Can Not Sign Electronic Consents • Process has not changed • Go to EDOCS • Print out forms that are needed • Consent to Treat • HIPPA • Whiteboard • Payment form • Stamp with baby card • Take to mother/family to sign • Place signed copy in chart • Provide copy if requested
