Corrective Actions • Cancel the standing, annual work permit for Complex LOTO (Completed). • Revise the C-AD LOTO Program OPM to require a specific written Complex LOTO procedure for all Complex LOTO (in progress) • Until C-A Complex LOTO OPMs are completed; require that Complex LOTO be covered by a job specific enhanced work permit (EWP) or equivalent control (completed)

  3. Corrective Actions • Update the C-AD LOTO Program classroom training course and conduct it every six months for all authorized LOTO workers until assessments determine that the LOTO program is functioning as expected at C-AD facilities, then conduct this training annually. Include training on how to determine when a Complex LOTO procedure is needed for C-AD specific equipment

  4. Corrective Actions • Develop a procedure in the C-A-OPM which describes the expectations of C-AD management for conducting and effective on-the-job (OJT) LOTO training program. This will address the trainer’s and the trainee’s responsibilities and ensure that OJT is uniformly conducted within the Department
