Fluid Initial Claims Application Layout Page 1: Welcome page for Florida Internet Claims Filing System (Select English) Page 2: Acknowledge/Consent to fraud statement (click submit) Page 3: Answer question “If you are filing an unemployment claim for yourself” (click submit) Page 3a: If the claimant answers “No” to question “If you are filing an unemployment claim for yourself” this page will populate give the claimant the option to exit. ****Depending upon the response to the question on page 3, an additional page will populate (page 3a) and the claimant will only have the option to exit the application entirely. **** Page 4: Enter residential zip code (click submit) Page 5: Acknowledge all (3) statements with “red asterisk” (click I agree) Page 6: Enter zip residential code, SSN#, answer question “Are you Unemployed due to Gulf coast oil spill” (click mouse here to proceed) Page 7: Answer all General Information questions with “red asterisk” (click mouse here to proceed) Page 8: Answer question “Are you the spouse of any of the following individuals” (click mouse here to proceed) Page 9: Answer all Personal Information questions with “red asterisk” (click mouse here to proceed) Page 10: Answer question “Please select the location from where you are filing this application” (click mouse here to proceed) Page 11: Indicate the nearest “Local office” from drop down box. (Click mouse here to proceed) Page 12: Answer questions for the “Identity Inquiry” with “red asterisk” (click mouse here to proceed) Page 13: Add Employer information. When complete (click finished adding Employers) OR (click no employers to report) Page 14: Enter Civilian Employer Information with “red asterisk” (click mouse here to proceed) ORIf no employers were added, the system will automatically proceed to page 14a. Page 14a: If the claimant selects “no employers to report” this page will read: “You have indicated that you did not work from 00/00/0000 thru 00/00/0000.(Click mouse here to proceed) ****Depending upon the reason for separation selected, an additional page will populate for the claimant to provide a detailed explanation of the separation (click submit) **** Page 15: Answer Eligibility questions with “red asterisk” (click mouse here to proceed) Page 16: Answer Employer payment Eligibility questions with “red asterisk” (click mouse here to proceed) Page 17: Answer Federal Income Tax question with “red asterisk” (click mouse here to proceed) Page 18: Select method of payment (click submit) Page 18a: If direct deposit is selected as the payment method, page 18a will populate.
