Purpose, Values, and Mission The core purpose of the Forum of ESRD Networks is: “To support and advocate on behalf of the ESRD Networks to facilitate improvement in the quality of care to patients with kidney disease” Five values have been identified as core for the organization. The core values that will shape the behaviors and actions throughout the organization include: Volunteerism, Collaboration, Innovation and Flexibility, Spread of Knowledge, Integrity, Autonomy of Individual ESRD Networks The Strategic Partners Committee is designed to bring together community leaders to discuss the mission and work plan of the Forum of ESRD Networks as it relates to the needs of the ESRD Networks, the current and projected climate of the nephrology community and CMS, and the opportunities for collaboration and service with partners/stakeholders.

  4. Proposed Members • 7 Community Representatives • 3 MAC Representatives These members will interact with a subgroup of the Forum Board of Directors. The first collaboration is scheduled for October 2008.
