  3. Principles of Self-Determination • Freedom to decide how to live life • Authority over a budget allocation • Support to organize resources in ways that are enhancing and meaningful • Responsibility for the wise use of dollars • Confirmation of the importance of self-advocacy

  4. Getting Started • IRIS receives a referral from the Aging and Disability Resource Center • Welcome call & packet • Choose consultant

  5. Building the Plan • Initial meeting and orientation • Identify goals and outcomes • Begin planning • Help the person find providers and think about provider agreements • Submit completed plan to IRIS Consultant Agency • Coordinate IRIS start date

  6. Additional IC Responsibilities • Create back-up plan • Ongoing contact with the participant • Help ensure continued eligibility • Prepare Allocation Adjustment and Exceptional Expense requests, if necessary

  7. IRIS Consultant Qualifications • Post High School degree or a minimum of two years of related experience • Strong verbal, written and interpersonal communications skills • Ability to transfer knowledge and understanding of the community to make positive community connections on behalf of the participants • Strong computer skills and the ability to learn new programs and software

  8. IRIS Consultant Qualifications • Access to a computer • Ability to travel with reliable transportation • Ability to work at least 20 hours per week and provide day, night and/or weekend hours • Ability to work on own with minimal supervision • Commitment to initial, monthly and ongoing learning opportunities

  10. IRIS Consultant Training • One-day orientation • Six online learning modules • Shadow visits with guides and mentors • Robust role plays • Final sign off from mentor • Twice-monthly conference calls • Ongoing learning opportunities
