  10. Keep Up-To-Date on Vaccinations There are many vaccinations to help keep our children healthy. If you have questions regarding what vaccines are available, give me a call.

  11. Over-The-Counter Medications All over-the-counter medications are to be provided by the parent/guardian. They will be locked up in individuals containers for your child/children with their names clearly labeled on the container.

  12. Prescription Medications All prescription medications given during school must be brought in by the parent/guardian, in the original container with the child’s name. A medication administration form must be signed by the parent and have the physician documented specific instructions on this form.
