Mississippi Health First’s mission is to address the incidence of health disparities and to increase health literacy and diabetes self-management to improve health outcomes.  The initiative will be characterized by a partnership between Federal and non-federal entities in order to maximize resources and affect the largest possible number of individuals with diabetes.  The collaborative partnership consists of members from the following Federal and non-federal entities:

  5. ● Administration on Aging (AoA) ● American Association of Diabetes Educators (AADE) ● American Diabetes Association (ADA) ● Centers for Disease Control and Prevention (CDC) ● Centers for Medicare & Medicaid Services (CMS) ● National Institutes on Health (NIH) ● Health Resources and Services Administration (HRSA) ● Housing and Urban Development (HUD) ● National Academy for State Health Policy (NASHP) ● HHS/Office of Minority Health (OMH)

  6. Additionally, the following organizations have been contracted by the Centers for Medicare & Medicaid Services to support this initiative: ●HCD International (HCDI) ● Healthcare Management Solutions (HMS) ● Information & Quality Healthcare(IQH)

  7. Mississippi Health First’s main goals are: ● to recruit and retain participants and providers; ● establish relationships at the local level; ● measure clinical outcomes; and ● improve the value of individual programs with a larger collaborative program. 

  8. What is unprecedented is its approach which includes the collaboration of Federal and non-federal partners as well as the hope for many additional partners, especially at the State and local level, including faith-based organizations.
