Incident/Concern Member of staff records the concern/incident on a Blue Incident Sheet and registers it in the CP log book Major Incident/Concern Immediately report to a DCPC DCPC consults other DCPC’s DCPC takes In-School action DCPC consults Social Services Duty Social Worker Governing Body regularly monitor Child protection via the CP log book DCPC read & summarises the report in the Incident Feedback file, dates & initials the entry in the CP log book DCPC consults ACPC DCPC checks/adds action to be taken, then dates and initials the Incident Sheet. This is then circulated to other colleagues on a “need to know” basis, via personal CP folders, which are placed horizontally in pigeon holes DCPC’s monitor the CP log book on a monthly basis to ensure all Incident Sheets are accounted for Decision not to refer to SS Decision to refer Less urgent referral Urgent Referral If action has not been carried out, or further action is needed, the DCPC returns Incident Sheet to key staff Colleagues read the report, initial , date and then pass it onto another colleague in the list, or back into the CP “IN” folder Decision made about whether to inform parents Parents not Informed Decision made about whether to inform parents Parents not informed If action is pending but dormant the Incident Sheet is temporarily held in a DCPC Pending Action File DCPC then checks that all the actions have been completed. Then signs and dates both the report and the CP logbook before filing it temporarily. Parents Informed Parents Informed Phone referral All Incident reports are then monitored before they are filed in pupil’s individual CP folders Initial Assessment & Referral Form Completed Incident Sheets are filed in a locked cabinet in Surgery Initial Assessment & Referral form faxed/posted/sent electronically to County Duty Service Folders may be accessed by colleagues on a “need to know” basis, via obtaining the keys from the DCPC’s
