  5. Conclusion: You Can’t Manage what You Can’t Measure • Measurement is the foundation for improvement. • Make measurement and data collection as efficient as possible. • Monitor results using a dashboard that all can see. • Report results and look for best practices.

  6. Questions to Consider when Reviewing Run Charts • When we will feel good about the care we are delivering? What is our goal? • What are the data telling us? What do these data say to the staff? • How confident are we that the data accurately reflect care being provided? • Do we care about this measure? If so, why? • All over the map? Look at the denominator size. • Steady increase? What did we do that worked? • Decrease? What changed? • What can we do to make it better? • Improved population management • Improved patient engagement, e.g. self-management support • Improved follow-up with patients, e.g., care management • Is the way that we are displaying the data helpful?

  7. Interactive Webinar • Four sites shared their measures, run charts, challenges & successes • <<List practice site names>> • I will provide an overview and then ask the site to share and respond to questions. • Ready, get set…unmute your lines!

  9. List of Measures for improving CAD care Coach: Have practice list measures, data sources, and frequency of reporting in the table above.

  10. Managing Lipids for our CAD Patients Coach: Insert run charts that show the value of reviewing multiple measures at the same time. For example, when assessing LDL levels, look at both the % of CAD patients with lipid profiles ordered and % of patients with LDL<100.

  11. Challenges & Successes Challenges: • Getting accurate registries • Low number of patients with CAD diagnosis • Expanded to include other diagnosis Successes • Meeting on a regular basis • Successful chart review to get baseline on 30 patients Coach: Have practice insert challenges and successes related to using these data for quality improvement.

  12. Coach: Repeat slides 8-11 for the other practice sites that are presenting.
