  1. Overview of the California LifeLine Process New Certification Customers • Customer calls their carrier and the carrier explains the program. If the customer claims they qualify, they are enrolled in LifeLine. • Carriers send a daily electronic file of new customers, customer updates, and disconnects/removals to Solix. • Solix generates list of customers to be certified based on the information provided by the carriers. An electronic file is sent to the printer. • The customer is mailed a certification form • The customer returns the form & any attachments to the PO Box, and they are electronically imaged (scanned). • The images are transmitted to Solix for review. • Decisions are sent electronically to the carriers regarding enrollment or disqualification of their customers. Simultaneously, Solix notifies its fulfillment center mail notification to the customer. 7 2 3 1 4 6 5

An electronic file is sent to the printer. • The customer is mailed a verification form. • The customer returns the form to the PO Box, and it is electronically imaged. • The images are transmitted to Solix for review. • Decisions are sent electronically to the carriers regarding the continuation or disqualification of their customers. Simultaneously, the fulfillment center mails notification to the customer informing it of the status of its continued eligibility. 5 1 2 4 3

An electronic file is sent to the printer. • The customer is mailed a certification form • Customer can go to the online website and complete their application process. • Decisions are sent electronically to the carriers regarding enrollment or disqualification of their customers.
