    2. 2 Purpose Of This Training As a Home & Community Base Medicaid Waiver Provider, one of your roles is to ensure the health, safety and well-being of all your clients. The purpose of this training is to instruct you on the following: How to document and submit a Critical Incident Report

    3. 3 Define Critical Incident A “Critical Incident” is any actual or alleged event or situation that creates a significant risk of substantial or serious harm to the physical or mental health, safety or well being of a waiver participant.

    4. 4 Which Critical Incidents Should I Report? Reportable Critical Incidents Are Defined As: Client Abuse; (Required to contact Florida Abuse Hotline 1-800-96 ABUSE or 1-800-962-2873) Client Neglect; (Required to contact Florida Abuse Hotline 1-800-96 ABUSE or 1-800-962-2873) Financial Exploitation; (Required to contact Florida Abuse Hotline 1-800-96 ABUSE or 1-800-962-2873) Client’s Death; Client’s Injury; Suicide Attempt; Client’s Arrest; Altercation; and Missing Adult

    6. 6 A Critical Incident Has Occurred….What Should I Do? 1st –If the Critical Incident is related to Abuse, Neglect and Exploitation, call the Florida Abuse Hotline @1-800-96-ABUSE or 1-800-962-2873 2nd - Contact Medicaid Waiver Specialist (MWS) 3rd- Complete “Critical Incident Report” and submit it to your MWS within 48 hours of the incident 4th- If your report was accepted by Adult Protective Services (Florida Abuse Hotline), please follow-up by asking them for the results of the investigation

    7. 7 Now It’s Time to Test Your Knowledge! Please complete the Critical Incident Report Provider Training Test. The test is located on Brain & Spinal Cord Injury Website: http://www.doh.state.fl.us/demo/BrainSC/Medicaid/medicaidhome.html. Select the Medicaid Provider Training option and choose the appropriate test. Once you have completed the test, please FAX your test to 850-921-0499 to the Florida Department of Health Medicaid Waiver Program. Your test will be reviewed and if you have a score of 80% or higher, you will receive a certificate of completion. Good Luck!!!
