Membership into National Honor Society is a privilege….not a right The key difference between Honor Roll and National Honor Society is that Honor Roll is met by grades alone…..National Honor Society membership includes scholastic, character, leadership and service components. You have met the scholastic and character requirements for NHS membership….now you must show how you fulfill the leadership and service requirements.

  3. Noblesse Oblige National Honor Society’s motto means that nobility obligates…in other words those of high rank have the obligation to behave honorably and responsibly to others

If you are selected into NHS membership you will be required to: • Pay $10 dues yearly • Turn in a signed Service Contract form yearly • Attend 6 out of 10 monthly NHS meetings and stay informed with the agendas and postings on the EVHS/NHS website • Complete and turn in a Service Hour Form per semester (5 Individual Hours & 5 Group NHS Service Hours are required per semester) • Maintain your GPA

Form # In order to make the process more objective, students will only be identified by a #, their name will not be submitted for review by the Faculty Council • Form # ______ • This is located on the top left hand corner of page 3 • Eligible NHS students need to fill in the designated # they have been given on ALL pages in the Student Information Packet.

Make a copy of your Student Information Packet Due to overwhelming demands, NHS Sponsors will not make copies of Student Information Packets for scholarship applications, service hour cords, etc. regardless of selection or non-selection • Please make sure you retain a copy of the packet for yourself before you turn it in to the NHS Sponsors.

  7. No late forms will be considered. Completion of this form does not guarantee selection. No late forms will be considered. Should it be determined that information on this form is incorrect or has been falsified in any way, the faculty council may rescind their invitation to apply or to join EVHS NHS, or they may dismiss a member without a probationary period. • Student Information Packets are due to one of the NHS Sponsors by 4:00PM on Tuesday, April 17th. No late forms will be accepted.

  8. Make sure your packet is complete. Legibly print or type all information on this form. Complete all sections of this form. Incomplete forms may be rejected by the selection committee and not considered.   9. Part I PART I: All activities require a signature of a sponsor, director or supervisor. A parent signature is not sufficient unless proof is provided that the parent is the sponsor, director or supervisor. Photocopies of certificates, letters or other forms of verification may be provided for proof of participation in lieu of adult sponsor signature. If a sponsor is no longer associated with the district or on the high school or 9th grade campus, see the NHS sponsors before the due date. Refer to pg. 9 for point designation. If you need more space, please attach an additional sheet, make sure to put your Form # in the top left corner and follow the table format. Fill in the chart for Part I • Years you have participated • Leadership positions Use pg. 9 to calculate points

  10. Part II Paid employment during the school year (not to include summer). Requires signature of supervisor or director • Follow guidelines on Part II for point designations • i.e. 1-5 hours per week=1 point; 21 + hrs per week =5 points

  11. Part III PART III-Community Service-This includes volunteer work performed during the school year and the summer. Service to consider includes church and civic organizations, Eagle Scout/Silver Award projects, tutoring (not a sibling), coaching non-school sports, participation in non-school drama/musical presentations, internships (not paid), voluntary service projects and other community involvement. Community Service hours must take place OUT OF SCHOOL day (i.e. HST hours at R.O.C.K. or PALS hours at various schools during school time MAY NOT be included in Part III). • Community Service • Must take place OUT OF SCHOOL DAY • i.e. PALS/Peer Buddies/HST hours occurring during class periods do not count towards Part III • 1 hr/week=1 point; 5 hrs/week=5 points

Part III Continued If you participated in volunteer work that did not occur regularly during the 32 week school year, estimate the number of hours you actually worked and spread them over a 32-week school year to assign points. The formula for this calculation would be the number of hours divided by 32. For these activities, please show that you divided the number of hours worked by 32. (Note-some point values may be in fractions -.5, .75, .33) • i.e. Mission Trip with Church Group to Mission Arlington (Taught VBS 15 hours during week of June 3-June 7) 15 hrs/32 weeks =.46875 =.47 points

Calculate the Points • Sum of Parts I + II x 50% • Sum of Part III x 50% • This can be completed by the NHS eligible student. • Be sure to double check your point totals for each part of the Student Information Packet • Total Points will be calculated by NHS Sponsors Only in the SPONSOR USE ONLY TABLE

Additional Info Page You can provide the Faculty Council with additional information that you believe would help the Council get a better assessment of your service and leadership traits. • For example, how participating in a service project has made an impact on you. • Awards you have received that are not included in the packet i.e. PTA Reflections contest winner

  15. Page 9 Point Values These are examples and certainly do not include all possible activities. If you have a question regarding an activity or participation, please see Mrs. Fischer or Mrs. Kramer prior to the packet deadline. • Co-Curricular (occurs during the school day-within school schedule ) i.e. Orchestra • Extra-curricular (occurs with school affiliation-not during school schedule) i.e. Student Council • Outside club/activity (not affiliated with EVHS) i.e. Girl Scouts, Boy Scouts

  16. StudentInformation Packets are due by April 17th at 4:00 PM Packets must be turned in to: Mrs. Stephanie Fischer, NHS Co-Sponsor (Room 424) or Mrs. Christina Kramer, NHS Co-Sponsor (Room 227) Good luck!
