UT FLEX Update • For Employees Only • Health Care Reimbursement Account The new annual maximum election will be $2,500 Everyone enrolled in the HCRA will receive a Debit Card at no cost Note: DCRA annual maximum remains at $5,000 IMPORTANT! Current plan year elections do not carry forward. Enroll in UT FLEX each and every year you plan to participate.

Steps to a Successful Annual Enrollment • Review OEB newsletters, handbooks, video presentations, and “Your UT Benefits Enrollment Options” letter • Attend your institution benefits meetings and fairs • Use My UT Benefits to make your tobacco-user or non-user declaration • Make elections no later than July 31, 2013 (including UT FLEX) • Review My UT Benefits Summary of Benefits before logging off • Complete and submit any required EOI forms by August 15, 2013 • Provide EOE documentation for dependents by August 15, 2013 • Review your first pay stub/benefits summary after September 1st for accurate deductions

The Living Well Health Platform powered by Provant! 15 minutes for your health Members will receive a $25 gift card after registering at the platform site, completing the Health Assessment, and completing your annual preventive exam all during this plan year. Getting started is easy. Review the Welcome Brochure in English and Spanishavailable at www.livingwell.utsystem.edu. If you have any questions please refer to our FAQs. Go to https://livingwell.provantonline.com Click on “Register New Account” and complete the registration. You will need your “Benefit ID or BID# to complete your registration. Go to http://bit.ly/utbid to retrieve your BID #. Submit your account Log in and complete your Health Assessment (under the Health heading) Enjoy your $25 gift card when you complete the Health Assessment and receive your annual preventive exam Complete both the preventive exam* and your health assessment no later than August 31, 2013. * No extra forms required - Provant automatically receives data confirming when registered UT SELECT participants have completed a preventive exam. Due to claim filing and processing requirements, it may take six to eight weeks for your preventive exam to appear on the Living Well Health Platform. UT System will accept claims postdated from September 2012. You will receive an email from Provant letting you know that you have completed both items and from that point you should receive your gift card within five to six weeks.
