  2. Personal Incidental (PI) Sick Days

  3. Benefit Entitlement 9 PI days per year Responsibilities Inform supervisor Maintain absence/present report Employee Entitlements and Responsibilities

  4. Roles To be informed by employee Responsibilities To keep track of employees’ illnesses Advise employee if doctor’s certificate is needed Annually review to look for patterns Supervisor Roles and Responsibilities

  5. Roles To consult with supervisor, as required Responsibilities To annually review absence/present reports To speak to supervisor if any patterns APO Roles and Responsibilities

  6. Short-Term Disability

  7. Benefit Entitlement On 3rd consecutive day 100% salary May be reintegrated part-time May send doctor’s note directly to APO or to Benefits Maximum of 5 PI days if same medical condition Responsibilities To provide valid medical certificate Certificates to be submitted on monthly basis If reintegrating, doctor’s note must specify days, hours, light duties if necessary To be available for expertise, if required Employee Benefit Entitlement and Responsibilities

Roles To manage absence To receive in a timely manner New information Extensions Return to work dates Replacement Costs Submit requests on a regular basis Responsibilities To ensure medical certificate is valid To send to APO or Benefits To follow up May need to post *No longer need to submit NSC Supervisor Roles and Responsibilities

  10. LONG-TERM DISABILITY (LTD) Entitlement, Roles and Responsibilities

Benefit Entitlement To apply for LTD To appeal if LTD declined Progressive return, if required Ends at age 65 Responsibilities To provide all necessary information To keep supervisor apprised of situation To comply with provisions of LTD policy, if accepted Employee Benefit Entitlement and Responsibilities

  12. Roles To be informed Responsibilities To fill out NSC To pay employee on miscellaneous form if on progressive return May need to post Supervisor Roles and Responsibilities

  13. Roles To consult with Benefits Responsibilities To keep supervisor informed To assist in reintegration, if necessary APO Roles and Responsibilities

  14. Roles LTD application Coordinate reintegration and progressive return to work schedules Meet with APO, Supervisor, rehab specialist, as required Responsibilities Maintain close contact with insurer Ensure close follow-up of cases HR Benefits’ Roles and Responsibilities
