Who is Eligible? • Employee Eligibility: • All regular, full-time hourly employees who work at least 30 hours per week. • Dependent Eligibility: • Your legal spouse • Your dependent children up to age 26* • Your dependent children of any age who become mentally or physically incapable of self care (physically and financially) before age 26*

Medical Plans • 100% coverage of eligible in-network preventive care services - no deductible • Limited coverage for Autism Spectrum Disorder • Expanded coverage for Women’s Preventive Benefits are being included under the medical and prescription drug plan. • No medical lifetime limit • No pre-existing condition exclusion for children under age 19 • Dependents up to age 26 eligible regardless of student or marital status

How to enroll • 2013 Open Enrollment Dates: September 16 – 20, 2013 • Election Paperwork Due on September 20, 2013. • Complete an Election Form if you want to: • Enroll in a plan for the first time; or • Make changes to your current plan choices • Complete an Evidence of Insurability Form if you are: • Enrolling in the voluntary life or long term disability for the first time • Increasing your current voluntary life coverage IMPORTANT: If you are not making any changes to your benefits, then no action is needed and your current coverage will automatically rollover to the 2013/2014 plan year.

When are Elections Effective Elections effective: October 1, 2013 Elections remain in effect from: October 1, 2013 – September 30, 2014 Can only make changes if you experience a qualified “life event” Your marriage, divorce or legal separation Birth or adoption of an eligible dependent child Death of your spouse or covered dependent child Change in your child’s eligibility for benefits Change in residence or work site that affects your eligibility for coverage Change in your work status that affects your benefits Change in your spouse’s work status that affects his/her benefits
