  2. Who we are: • Barbara Jenkins • Head of Clinical Planning • Keith Broughton • Data Quality Improvement Manager • Natalie Smith • Partnerships & Involvement Manager

What we have been doing since last year • Re-organisation of responsibilities within the Trust led to the appointment of: • Jude Archer – Deputy Director Governance • Lead Directors responsible for each standard is now well established within the Trust. Directors have systems in place to regularly review evidence & levels of compliance

  4. C6 : Cooperation between health & social care to meet patient needs • Assessment – Compliant • Evidence available includes: • Multi agency Children’s Safeguarding unit opened at the Lister Oct 2008 • Integrated Transport Project Board established with PCT, HCC to look at solutions for travel to health facilities across the County. • Multi agency Safeguarding vulnerable adults group with explicit Terms of Reference includes Police, HPFT, ACS, CPS & PCT.

C13a Systems in place to ensure patients, relatives & carers treated with respect. • Assessment : Compliant • Evidence available includes: • Trust Objectives & mission statement • Patient satisfaction survey • Privacy & dignity training (one off and at Induction) • Patient Experience Strategy 2008 • Joint meeting with PCT re complaints, audit & patient experience

  6. Timetable Final Annual Health Check Declaration: • 3rd April • To Trust’s Risk and Quality Committee • 29th April • To Trust’s Board • 1st May • Healthcare commission/Care Quality commission

  7. C13b systems in place re appropriate consent • Assessment – Compliant • Evidence available includes: • Consent policy (reviewed April 08) inc. Explicit arrangements re competencies to take into a/c pt needs, languages & the Mental Capacity Act. • ENH Safeguarding Children Policies & Procedures • Range of specific consent forms & competency framework • Consent audit undertaken 2008, findings shared with staff

C13c systems in place to ensure confidentiality patient information • Assessment – Compliant • Evidence available includes: • Range of policies inc Information Security, Confidentiality code of practice, records management • Audits to review compliance • Staff leaflets/cards on info security • ENH Safeguarding Children Polices & Procedures, Info shared, database kept by Named Nurse.

C17 views of patients, cares & others taken into a/c in design, planning, delivery & improving care • Assessment – Compliant • Evidence available includes: • DQHH consultation report & presentations to topic group. DQHH Involvement plan developed jointly with PCT, inclusive & all phases. • Foundation Trust consultation, engagement plan and report • Review of Trust’s car parking policy initial consultation & review of implementation • Patient Experience Strategy & Implementation plan

  10. C18 Equity of access to services & choice. Equitable treatment. • Assessment – Compliant (wef Aug 08) • Evidence available includes: • Equality Impact Assessments now part of standard practice & staff training commenced Nov 08 • Introduced Learning Disability Facilitators with HFPT to assist with planning services to meet patient needs • Renal expansion includes the development of home dialysis service increasing choice & access • Involvement Strategy & Action plan reviewed Nov 2008

C22a&c promote, protect & demonstrate improving health of community & narrowing health inequalities • Assessment – Compliant • Evidence available includes: • Active member of County & Local LSP’s, recent actions supported the allocation of funding to exercise for cardio-rehab patients, breast feeding initiatives, promotion of cycling proficiency for adults • Introduction of Learning Disability Facilitators • Trust Security Manager member of Herts Local Crime & Disorder Reduction partnership
