  2. School-Based Health Care • Garfield Elementary School • Gifford Elementary School • Highland Elementary School • Sheridan Elementary School • Every Wednesday, alternating locations, 3:00 – 6:00 pm

  3. Diana Uselding, CPNP Certified Medical Assistant Receptionist Health Benefits Specialist School Physicals (57%) Well Child Exams (27%) Lead Screening (10%) 216 unique patients 255 medical diagnosis An additional 175 students received flu shots Staffing and Services Provided

  4. Select Patient Demographics • 51% Female • 69% Hispanic or Latino • 12% African American • 13% Caucasian • 37% either 5 or 6 years old • 20% either 10 or 11 years old • Have provided care to ages 0 - 18

GEFCC Would Like To: • Secure authorization to explore increasing frequency at Garfield, Highland and Sheridan with each school’s leadership • Secure authorization to explore and add school-based health care service provision at additional schools, to start this coming academic year • Lords Park • Channing • Parkwood • Ontarioville • Secure authorization to explore receiving referrals for behavioral health services from District personnel, with services provided at our Health Center locations • Encourage consideration of a fixed, permanent school-based health center, in partnership with potential funders such as the United Way of Elgin

Secure Authorization to Explore Increasing Frequency at Garfield, Highland and Sheridan with Each School’s Leadership • Permission to speak with the leadership at each school • Desire to increase frequency up to weekly if need exists • Anticipate offering the same services as we have, only at an increased frequency • No change for the School District, other than use of District facilities more frequently

Secure Authorization to Explore and Add School-Based Health Care Service Provision at Additional Schools • Would potentially start as early as this coming academic year • Lords Park, Channing, Parkwood, Ontarioville • Would like permission to speak with their leadership and initiate exploration • Projected scope of services would be identical to what GEFCC has currently been providing in District schools

  8. Secure Authorization to Explore Receiving Referrals for Behavioral Health Services from District Personnel, with Services Provided at our Health Center Locations • Permission to begin exploration with District personnel • Collaboration would include Family Service Association of the Greater Elgin Area • Envision use of Social Workers, Psychiatrists and Primary Care Providers to offer services matching District IEP needs: ADD/ADHD assessment and treatment, medication monitoring & education, identification of co-morbidities and referrals, etc… • Envision services provided to District students at no cost to District – likely all services will be funded by All Kids or commercial insurance

  9. Encourage Consideration of a Fixed, Permanent School-Based Health Center, in Partnership with Potential Funders Such as the United Way of Elgin • GEFCC would be happy to staff a committee to explore this possibility and return with recommendations • Funding from the United Way of Elgin is a possibility • GEFCC has already initiated the process of making ourselves eligible for funding from IDHS for school-based health care
