    4. PMC Index Case Client J. J. Born 7-31-1959 (in 40’s) HIV Medications Truvada 1 tablet a day Reyataz 200 mg capsules, 2 per day Norvir 100 mg capsules, 1 per day Viral Load <50 on 3/2/06 CD4 Count 304 and 37% on 3/2/06 Panic attacks – MHMR client Alcoholic – AUDIT score 10, been to AA

    5. Colon History 5/14/02 Gross rectal bleeding. Positive family history for CA colon. GI consult requested. 8/11/03 Anal abnormalities on exam and continued bright red bleeding. GI consult arranged. 4/15/04 Four anal surgeries for recurrent anal cancer. Radiation 39 treatments Chemo 39 treatments 7/23/04 Abdominal perioneal resection and colostomy

    6. Cancer History (Continued) 2/16/2006 Complains of neck and left shoulder pain 3/2/2006 MRI 2.3 cm mass in T-2 and soft tissues. Saw oncologist for metastatic cancer 3/17/2006 Increased pain Pain treatment with MS Contin 6/20/06 Died of metastatic anal carcinoma
