  3. Background • RFB: RAF/2012/00029: Appointment of a service provider to provide Funeral Benefits Scheme for the Road Accident Fund (RAF) employees for a period of five (5) years. • Advertised on 30 November 2012 • Closing on 21 January 2013

  4. Background • The RAF wishes to appoint a suitably qualified Service Provider to provide a Funeral Benefits Scheme for RAF employees for a period of five (5) years.

  5. Mandatory Documents • Standard Bidding Document (SBD) • Tax Clearance Certificate • Bid Conditions • BEE certificate

  6. Evaluation Criteria Evaluation will be based on the following: • Technical Functional Mandatory (Comply or Not Comply) – Bidders who do not meet the mandatory requirement will be disqualified. • Technical Functional Requirement (100 points) • Price and BEE Evaluations (90/10 points)

  9. Disqualification of bids • Bidders shall be disqualified if they fail to: • Submit a valid and original tax clearance certificate • Complete questionnaires in full • Submit true and correct information • Comply with mandatory requirements • Comply with prescribed response format

Bid responses sent by courier must reach the reception at least 36 hours before the closing date to be deposited into the bid box • Submission Register must be signed at the reception by bidder when submitting bid documents. Important note: • Please ensure that the attendance register has been signed • Name of company • Contact details • Late bids will not be considered Submission of bid responses

Purpose of the RFB • The RAF currently has a Funeral Scheme in place; • The benefits by the current service provider do not cover parents and parents-in-laws above the age of 75; • The purpose of the RFB is to improve on benefits by the current service provider and also include acceptance to the scheme for parents and parent-in-laws who are older than 75.
