Causes of failure in action plan Haven’t got good definition (clarifying) of problem or activity (situation analysis) Insufficient Skill of staff about design an action plan Haven’t got right standard & monitoring and evaluation indicator Haven’t got specific audience Lack of stake holder participation Insufficient Supervision & intervention of manager or upper level on lower level Unpredictable program (from national level or provincial level)

  4. Goal The goal of a plan summarize what is to be achieved by the action in define time & place and for special audience in general terms. General Audience base Time bound place
