  2. Center for Pediatric Medicine Primarily serves Medicaid patients in Greenville County Patients are seen by Residents with Supervising Faculty or Providers (MDs, NNPs, PA) Two associated clinics North Greenville Pediatric Rapid Access FY 2011 16, 547 patients 46,192 visits

  3. Center for Pediatric Medicine 2006: RRT, AE-C (Cheryl Kimble) came to help with PFTs, treatments, and education part-time Medical Home Asthma Initiative Meeting in October 2008 March 2009, brought her on full time for case management of all the asthmatic patients

Asthma Action Team Clinic 3 half day clinics per week for asthma Staffed by an MD and RRT/AE-C Bilingual, multidisciplinary, family-centered asthma education Spirometry Case Management Asthma registry Alert system for ED visits and hospitalizations Medication tracking Communication with schools and daycares Phone calls Flu vaccine Pictorial Asthma Action Plan

Greenville Pediatric Asthma Community Collaborative (GPACC) Collaborative involving Greenville Hospital System Pediatric Practices, Greenville County Schools, BlueCross BlueShield Foundation of SC, BlueCross BlueShield of SC, Merck, and the SC Asthma Alliance Goal is to improve the care of children and adolescents with asthma in our community Reduction in hospitalizations Reduction in emergency room visits Decreased school absenteeism Decreased work absences for caregivers Implementation of Asthma Action Plans Improved quality of life (measure through Asthma Control Test scores)

Greenville Pediatric Asthma Community Collaborative (GPACC) Intensive case management/asthma education program Home visits School visits Coordination of care visits involving the primary care physician Monthly phone calls Tracking of medication compliance Current Enrollment (targeted population based on hospitalization and ED utilization) 31 active participants (11 enrolled in last 3 months) All are Medicaid or uninsured Outcomes so Far No hospitalizations 1 emergency department visit for asthma Quality of life scores demonstrate improvement 100% with Asthma Action Plan 100% on controller medications
