What is the HealthyExtensions Program? • A value-added discount program that provides: members access to various alternative health care providers & services that promote & enhance healthy living • A program that will allow Blue Cross of California members to: take advantage of discounts ranging from 10 to 50 percent on a wide variety of health & wellness products & services

  3. Is there a charge for this program? • NO. There is no additional premium cost to our members! • The products & services in the catalog: are not covered benefits under the members’ health plan. Costs of these program services & products do not count towards calendar year co-payments maximum, lifetime maximums and/or deductibles

  4. What vendors are participating in the program? • Blue Cross Health & Wellness Programs are part of the HealthyExtensions program *MedCall *Check Up Centers - PPO only *Prenatal Program *Health Management Programs *(usual policy & procedures will apply) Click here for additional vendors

  5. What vendors are participating in the program? • 24-Hour Fitness Health Club memberships and services including personal trainers. Discounts of $50 to $100 off enrollment fees & 38% discount for a personal trainer • American Specialty Health & Wellness Mail order health & wellness educational material, including books, newsletters, videotapes, yoga & aromatherapy products at an average discount of 15% Click here for additional vendors

What vendors are participating in the program? • Essentials by Ethical Nutrients A special line of nutritional & vitamin supplements & herbs at discounts up to 50% • EverLife Store Wellness products including a wide range of books, audiotapes & videotapes regarding health & fitness at discounts of 15% to 30% Click here for additional vendors

What vendors are participating in the program? • Home Fitness Club A Lifestyle program concentrating on fitness lifestyle changes at a discount of 33% • Pritikin Interactive System Lifestyle program concentrating on dietary lifestyle changes at a discount of $10 to $30 off the standard price. Practitioners that are selected for the HealthyExtentions networks are screened for certification, training, and educational requirements established by professional organizations in these disciplines • Massage Therapists • Yoga Centers • Hypnotherapists With a discount of 10% to 25%

  9. Who is eligible to participate in the program? • All Individual & Small Group members who are enrolled in Blue Cross medical plans

  10. How are members notified about HealthyExtensions? • Announcements appear in The Advantage newsletter for current members • HealthyExtensions brochures are included in the new member enrollment kits

  11. How do members receive the HealthyExtensions catalog? • Call the Customer Service number on their member ID card to request a free catalog • Complete the business reply card in the new member enrollment kit and drop it in the mail
