  3. Select patient’s name

  4. Go to Risk Clinic

  5. Diseases for the selected patient

  6. Genetic testing information

  7. Change group to see testing panels for different syndromes

  8. Breast/Ovarian

  9. Cardiac

  10. Upload relevant documents

  11. Details about the selected patient

  12. Ways to add a complete family history: 1. By the patient: - Tablet PC - Web based questionnaire 2. By the doctor: - Upload from another software package (HL7 message) - Family history Tab/ Bulk Add Function

  14. Bulk Add Function (Add relatives all at once)

  15. Select the number of brothers

  16. Select the number of sisters, sons, daughters

  17. Select the number of Aunts and Uncles on the mother’s side

  18. Select the number of Aunts and Uncles on the father’s side

  19. Click Add Relatives

  20. Go Back to the Pedigree View

  21. Complete Pedigree with all the information added

  22. Select the patient’s sister and add new disease

  23. Change group to see diseases for different syndromes

  24. Change group to see diseases for different syndromes

  25. Change group to see diseases for different syndromes

  26. Select one disease

  27. Type in any two letters for a disease will bring up a list. Select one

  28. Add the age

  29. Add details for the patient’s sister

  30. Add Name Add Age Add Vital Status

  31. Pedigree is automatically updated with the new information

  32. You may also add new disease by right clicking on a selected member of this family

  33. Right click

  34. Select Add Disease

  35. Select one disease

  36. Type in any two letter and select a disease

  37. Add Age

  38. Click Add

  39. Complete Family Pedigree
