  3. HEALTH /DENTAL/VISION INSURANCE Last Day to Enroll: September 12, 2013 Effective Date: First of the month following date of hire if paperwork submitted to Human Resources by 9/12/13. After September 12, 2013 You must wait for open enrollment Effective Date: January 1, 2014

  4. Life/Long Term Disability Last Day to Enroll September 12, 2013 Effective Date Life/LTD: Date of hire if paperwork submitted to HR by 9/12/13. After September 12,2013 You must go through evidence of insurability and issuance of coverage is at discretion of Mutual of Omaha

  5. Who is an Eligible Dependent? • Legally recognized spouse • Domestic Partner*additional paperwork required-contact Tracy Thompson at Ext 5017* • A child under age 26 (age 25 for Life Insurance) • Dependent child over age 26 incapable of self-support due to disability

  6. We will accept any of the following documents as proof of dependent status. • Marriage License • Divorce Decree • Common Law Documentation • Domestic Partner Affidavit • Birth Certificate • Adoption Decree • Proof of Placement while adoption proceedings are in process • Court Order (Qualified Medical Support, or otherwise) • Legal Guardianship Documentation

Plan Changes • Mid-year changes to the Medical, Dental, Vision and Flexible Spending Accounts can only be allowed within 30 daysof a Qualifying Event. • Some examples of a Qualifying Event are marriage, birth/adoption of child, loss of coverage or eligibility for coverage on a group plan. • Contact Human Resources

  8. Basic Term Life Insurance Mutual of Omaha $25,000 Coverage Or 1 x Annual Salary Whichever Is Greater No cost to the employee.

  9. Voluntary Additional Mutual of Omaha $25,000 Or $50,000 Or 1 to 4 x Annual Salary Term Life Insurance

  10. Evidence of Insurability Is Required: • If you apply after your first 30 days of employment. • If the total of your voluntary coverage is over $325,000. Is Not Required: • If your total coverage increases to more than $325,000 as a result of salary increases. Maximum coverage amount: $800,000.00 Basic and Voluntary coverage combined

  11. Voluntary Dependent Life Mutual of Omaha Spouse- up to 50% of Employee Voluntary Life Maximum Benefit $50,000.00 (multiples of $5,000.00) Children- Maximum Benefit $10,000.00 (multiples of $1,000.00) Employee must purchase Voluntary Life to purchase Dependent Coverage.

  12. Long-Term DisabilityMutual of Omaha Benefits Option 1 Option 2 or: 70% of salary up to $10,000 per month 60% of salary up to $10,000 per month • Benefits start after 26-week elimination period Existing annuity contributions continued

  13. Long-Term Disability Evidence of Insurability • No Evidence of Insurability is required within first 30 days of employment. • After first 30 days of employment, Evidence of Insurability form must be completed and issuance of the coverage is at the discretion of Mutual of Omaha.

  15. AFLAC • All AFLAC premiums except Short Term Disability are deducted pre-tax. Short Term Disability premiums are deducted after-tax. AFLAC pays cash directly to you, above, beyond, and regardless of any other coverage • Enrollment occurs within the first 30 days of hire or at open enrollment

  28. Employee and dependents are eligible to carry coverage for 18 months if the employee terminates or has a reduction in hours. Other than the above occurrences, covered dependents who are no longer eligible to continue coverage can carry coverage for 36 months. C O B R A Continuation of medical coverage after termination or change in job or dependent status. Please notify us when these events occur.

  29. You must complete 10 years of employment in a regular position. You must be at least 55 years old. Your age plus your years of service must equal 75. To Be a TCU Retiree You Must Meet the Rule of 75:

Vacation/Sick Leave • Faculty do not accrue vacation time. • Those with less than 6 years of employment can use up to 3 months sick leave with pay & 3 months extended medical leave without pay. • Those who have 6 years or more of employment can use up to 6 months sick leave with pay & 6 months extended medical leave without pay. .

  34. Tuition Assistance Employee: The tuition benefit is defined as financial aid and is contingent upon satisfactory academic progress as determined by the financial aid office. • Six-month waiting • period from date of hire • Limited to 6 credit • hours per semester

  37. Alternative Schooling If your dependent child is denied admission to Texas Christian University, the University will reimburse community college tuition for Tarrant County College or a community college in a county adjacent to Tarrant County.

  38. Tuition Exchange • TCU participates in the tuition exchange program. • Three year waiting period from date of employment. • If your child decides to go to another college other than TCU, they can choose from a college that participates in the tuition exchange program.
