  1. ABILITY OF OCT TO IDENTIFY RNFL DEFECT BEFORE STANDARD AUTOMATED PERIMETRY IN GLAUCOMA Orna Geyer, Tagil Arev-Fishelzon, Eitan Segev Carmel Medical Center, Haifa

  2. In glaucoma, structural changes in the optic disc precede functional visual impairment

  3. Purpose • To evaluate the capability of OCT for detection of nerve fiber loss before the appearance of a defect on visual field examination in glaucoma

  4. VF OCT Methods • 29 glaucoma eyes with altitudinal VF defect • 29 healthy eyes of age-matched normal individuals

  5. OPTIC DISC OCT VF Temporal nasal Correlations between retinal nerve fiber layer and visual field in eyes with nonarteritic anterior ischemic optic neuropathy. Deleon-Ortega J. et al, Am J Ophthalmol. 2007

  6. OCT- normative database

  7. 17.2% 58.7% 24.4% RNFL Thinning in the Quadrant Corresponding to the Unaffected Hemifield

  8. Superior Quadrant 125 m 100 RNFLT 75 50 25 Glaucoma Normal Glaucoma Normal OCT OCT Glaucoma / Our Normal Control

  9. OCT Normal Glaucoma Inferior Quadrant 200 150 m RNFLT 100 50 0 Glaucoma Normal OCT Glaucoma / Our Normal Control

  10. Effect of the Depth of VF Defect

  11. 10 Depth of VF defect / Thinning of RNFL in the Quadrant Corresponding to the Unaffected Hemifield 8 6 No. of eyes 4 2 0 -10 -15 - 20 -25 -30 MD (dB)

  12. Take Home Message • OCT can recognize RNFL loss beyond a corresponding VF defect • OCT may be more sensitive than VF test for the detection of the extent of the glaucomatous damage
