The Tool Includes Two Types of Questionnaires • 11 Age-Specific Questionnaires Concerning Child Development • A Questionnaire About Those Early Life Experiences or Risk Factors That Predispose Children to Having Disabilities or Other Special Needs. If The Child Was PrematureThe Discovery Tool Adjusts for Prematurity • Definition: A baby born before 37 weeks gestation (the time from conception to birth) is considered premature; when born before 32 weeks, a child is considered significantly premature. • Adjustment for Prematurity is made automatically online by the Discovery Tool. Functional Skills • Basic Functional Skills Which Scientific and Clinical Research Indicates the Average Child of a Certain Age Will Develop and Demonstrate • Examples: For a 4 to 5 month old child Body Movement - Physical Development and Motor Skills, such as: “Does baby push ups” or “Brings hands and toys to his or her mouth” The CICC Discovery Tool and Referral System DESC9

Potential Problem Behaviors • Behaviors That May Interfere with a Child’s Ability to Learn and to Get Along with Others When They Occur or Do Not Occur During Certain Age Periods • Examples: For a Child in the 2 year, 6 month through 2 year, 11 month age period Language Behaviors such as “Staring into space or talking to self a lot” Or “Not speaking at all” The CICC Discovery Tool and Referral System DESC10

SCORING SYSTEMCHILD MAY HAVE SPECIAL NEEDS IF USER INDICATES • An Absence of Four or More Functional Skills From All Areas of Development, or • An Absence of Two or More Functional Skills from One Area of Development, or • The Presence of Three or More Potential Problems From All Areas of Development, or • The Presence of Two or More Risk Factors Conditions The CICC Discovery Tool and Referral System DESC12
