  2. The W-4 - (1 PAGES) The W-4 is a tax form, in this instance, used by your employer to report financial information to the IRS. They are used to report income and calculate taxes to be paid to the federal government of the United States. Personal Allowances Worksheet: When you complete this worksheet it creates a measuring stick to calculate how many allowances you have (as an earner) and how much of your earnings will be withheld to compensate for taxes due in April. Employee’s Withholding Allowances Certificate: The W-4 asks you to provide basic information here. LINE 5: Report you total allowances from H on the worksheet. LINE 6: Write “0” on this line unless you would like to have any additional amount withheld from your checks that will be returned when you file your taxes. LINE 7: Many youth find that they are exempt from paying federal and state income tax based on their income. Read over the instructions circled above and decide if you are eligible to write “exempt” here. • Click Here for help!

  3. Work Permit Applications For Minors- (2 PAGES) A work permit is issues by the school (district) to ensure the minor is enrolled and maintaining a certain grade point average to work. In addition, it specifies the number of hours and type of work a minor can do to protect the minor and the employer. WE NEED YOUR WORK PERMITS ON THE DAY OF YOUR ORIENTATION !! TeenForce will provide you with 2 applications – 1 for TeenForce and 1 for the employment site where you will be performing your work. ALL SIGNATURES MUST BE ORIGINAL SIGNATURES! (no copies, scans, or faxes.) Fill out your basic information. Have the employer sign here. Have your parent or guardian sign here. Once you have all original signatures, take them to your school or school district where they will issue you 2 work permits. Bring both back to TeenForce on the day of your Orientation.   4. The I-9 - (5 PAGES) In order to work in the U.S. the government requires this form to be filled out and kept on file by your employer. The circled the area needs to be filled out by you and brought back to us. Don’t forget to sign and date it! A TeenForce rep will fill this part out. You will need identification document(s) to verify your eligibility to work in the United States. Accceptable documents are listed on page 5 of the I-9. Click Here for help!

  6. The Employee Agreement – (4 PAGES) This is your employee agreement between you and us (TeenForce). There are two versions depending on whether you are over 18 or not, which requires a parent signature. Some main points are… LINE 2:You are authorizing TeenForce to run a general background check on you . LINE 5:Your agreement to do the best job you can while on the job as a TeenForce candidate and other general expectations of our employees. LINES 6 & 7: We can't guarantee that you're going to like the job – we’re here to support you and make it a valuable experience. It is also to remind you that you are a TeenForce employee, not an employee of the Client . LINE 8 & 9: You do not have the authority to make decisions on behalf of TeenForce or the Client. LINE 12: You are not eligible for benefits from the Client (medical, retirement, holiday etc.). LINE 13:You are not allowed to drive as part of your job. (You may drive to and from the job). LINES18 &19: Legal wordage stating that you will not place blame on TeenForce for incidents that occur at the Client's location and Vice-versa. Click Here for help!

  7. The Background Check Disclosure and Authorization Form – (1 PAGE) This document expands on the Line 2 of the Employee Agreement and tells you what kind of information may be disclosed. It also states you have the right to obtain a copy of the Background Check. Click Here for help!

  8. The Acceptance of Assignment – (3 / 4 Pages) In this document you are acknowledging the details of your work assignment. If you are over 18 your AOA is without parent signatures. Exhibit A: Spells out the details for your assignment(s) such as the name of the Client, Job Title/Description, Shift, Location (of where you will be working), and the Pay Rate. Exhibit B:States that as an employee you do not have medical or other benefits (such as paid time off). Exhibit C:Reinforces the importance of confidentiality in working with TeenForce and the Client. Exhibit D:(Only for minors) asks for parents/ guardian to acknowledge your work assignment.   9. The Medical Provider Network Implementation Notice – (1 PAGE) The MPN is an entity or group of health care providers set up by an insurer or self-insured employer and approved by Division of Workers Compensation to treat workers injured on the job. This notice let's you know that in the event of a work related illness or injury, TeenForce has coverage and you may be asked to see a doctor within our network instead of your personal doctor. In the supplemental orientation documents (description on the next slide) there is more information about the MPN. Click Here for help!

  10. Acknowledgement of Documents Received – (1 PAGE) This form asks you to acknowledge that you understand where you can find each of these documents. We are required to provide you with access to them, but there are for your information only. They range in topic from sexual harassment to CA state benefits. Click Here to access these documents.
