Change Management The purpose of Change Management is to ensure: “The continued and uninterrupted service of the Live environment through a controlled, proactive and documented process”. • This goal is achieved by enforcing a strong, effective Change Management procedure that provides both visibility AND accountability.

  8. Change Request - Minimum Data Set • What’s Changing? (detail please) • Why should this change be done? • What do you expect the outcome of this change to be? (also – what will happen if you don’t do this change?) • Who is going to do this change? Who will notice? • Where is this change going to happen? • What system will be changed? • Is there a system Outage required ? • How long will the change take? • When will the Change happen? (date & time) • How urgent is this change? • What else will be affected by this change? (e.g. Impact analysis?) • What can go wrong (likelihood and impact) - e.g. Risk Assessment • Detailed implementation plan including details of implementers • Detailed back out plan including details of implementers and timescales • Detailed test plan including testing sign-off if required
