  2. Fatal #1 California • January 21 (Monday) • Slip and Fall of Person • Cement operation • Age 58 • Contractor Truck Driver • 3½ years experience

Fatal #1 California • He was found on the ground behind the rear tires of a bulk cement truck that was located at a truck access rack. • Truck drivers used this facility to access the hatches on the top of the trucks. • Although there were no eyewitnesses, the victim's injuries were consistent with a fall from an elevated location.

Fatal #2 Missouri • The victim was hit on the head by a pry bar that fell from an 80-foot high elevated work platform. • He was wearing a hard hat at the time of the accident and was bent over shoveling. • The pry bar struck him behind the hard hat. • The victim was hospitalized and died of his injuries later that day.

  7. Fatal #3 Nevada • The victim parked his haul truck near an intersection of the loading area and left. • A short time later, another miner got in the parked truck to move it out of the way. • When this driver was backing the truck, it struck the victim as he was returning to his truck.

Fatal #5 Wisconsin • He was using an excavator to free a dragline from an ice-covered pit floor. • The excavator broke through the ice as it traveled over an ice-covered, 10 feet deep ditch towards the dragline. • The victim was ejected and was recovered from the water about six hours after the accident.

  13. Fatal #6 Nevada • The victim and two other miners were performing maintenance on a roof bolter when cemented backfill fell, covering him. • One of the other miners was injured and hospitalized.

Fatal #8 Kentucky • The victim was working alone attempting to clean a clogged drainage ditch. • He was operating an excavator on the water-covered pit floor. • One of the crawler tracks fell into an 8-foot deep sump hole that was not visible due to the high water. • The excavator fell on its side, trapping him inside the cab.

  19. Fatal #9 Texas • The feeder portion of the crusher clogged while the victim was dumping rock in it with the excavator. • The victim remotely shut off the feeder but left the crusher and discharge belt running. • He climbed onto the feeder to check it and fell into the crusher.

  21. Fatal #10 California • The victim backed a truck to the edge of a stockpile to dump. • The truck went over the crest and fell approximately 30 feet to the floor below.

  23. Fatal #11 New York • The victim was using a jackleg drill to drill a hole near the face when the bit came off the drill steel. • He was attempting to retrieve the bit when a piece of rock, approximately 8½ feet x 4 feet x 14 inches thick, fell from the roof striking him.

Fatal #12 New Mexico • The victim was electrocuted when he contacted an energized steel water line. • One conductor of a 480-volt electrical circuit short-circuited to an abandoned heater tape where both circuits passed through a common conduit box. • The heater tape, which was attached to the water line, overheated and energized the water line to 277 volts.

  27. Fatal #13 Arizona • The victim was working on a 480-Volt floodlight at the tailings booster station. • He contacted energized electrical wiring supplying power to the light.

  28. Fatal #14 Pennsylvania • August 18, 2008 (Monday) • Machinery • Sur of UG Stone Operation • 28 year-old • Mechanic / Welder • 8 years experience

  29. Fatal #14 Pennsylvania • A crane was being used to lower a 7,000 pound counterweight for a scaling machine. • The victim was checking alignment of the bolt holes for the counterweight. • One of connectors on the rigging broke, causing the counterweight to swing and strike him. • The second connector then broke and the counterweight fell - striking the victim.

  31. Fatal #15 Arizona • The victim had been instructing persons making a belt splice. • He fell through a 4-foot by 6-foot opening - 28 feet to the floor below.

  33. Fatal #16 Texas • The victim was traveling on a mine road in his personal vehicle. • It traveled through a berm and into a body of water.
