The Patient-Centered Medical Home Defined Personal physician- first contact, continuous and comprehensive care Physician directed medical practice - team provides ongoing patient care Whole person orientation - provides for all patients’ health care needs or arranges care with other professionals. Includes care at all stages of life (acute, chronic care and end-of-life care and preventive services) Care is coordinated and/or integrated - across health care system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient’s community. Care facilitated by registries, information technology, health information exchange. Care provided when and where needed.

NCQA Recognizes Health Systems WHO: • Improve patient access • Adopt systems that improve care for chronically ill and • high risk patients • Measure clinical effectiveness (UF&Shands currently • manages 11 specific disease management programs) • Enhance the patient satisfaction experience • Improve patient outcomes • Deliver cost savings (short and long-term)

  4. NCQA Results in Recognition Levels Level 3: 75+ Points; 10/10 Must Pass Level 2: 50-74 Points; 10/10 Must Pass Level 1: 25-49 Points; 5/10 Must Pass Increasing Complexity of Services

UF&Shands Primary Care Network Profile • 23 Total Primary Care Clinics • 6 clinics are NCQA Level 1 Medical Home certified • 2 Disparity Clinics • Services Offered: • Pediatric/Adult well visits • Immunizations • Chronic disease care • Acute care illness • Prenatal care • Dermatological procedures • Weight loss counseling • Minor surgical procedures • School physicals • Care for aging population

The Patient-Centered Medical Home • ASO and Provider Service Networks • Provider Service Networks are community focused • Allows for more $ to be used for the actual provision of care • (medical loss ratio) • Creating a single network or organization that cares for nearly all of patients needs • Theorectically should result in stabilizing costs • Current primary care networks serve as a gateway to the model
