  2. Caring for the dying • 500,000 Deaths per year • 58% in hospital • Physical symptoms, anxiety, depression, social and spiritual difficulties. • Care needs of the family • Proper management requires collaborative, MDT working

  3. For a death I’d give it a 7/10 • Identify • Provide care • Dignity, Comfort, Family, Home, Prayer, Pain Free, Quick • Traditional approaches vs using a pathway

  4. Audit Cycle – What we did • 2 centres • YsbytyGlan Clwyd and Ysbyty Gwynedd • Retrospective audits over 3 months • Cases identified through coding • Pre – introduction (53 cases) • Post introduction (31 cases)

  5. Results • Improvement of documentation in all areas • DNAR Documentation 53% to 100% • Discussions with family 97% to 100% • Instruction for care • Discontinued meds 39% to 81% • Palliative medication 27% to 90% • Withdrawal of organ support 57% to 87% • Referral for organ donation 11% to 87% • Improvements in spiritual and bereavement care
