SAFETY ALERT No. 2005.4 Fall Injury – arm fracture, work on sloped/wet surface What happened: A concrete mixer operator was cleaning non-operating concrete mixer machine, while descending his left foot slipped due to wetness and slope of the hopper, resulting in his fall on the ground and fractured his upper left arm. Learning Points: • Recognize worksite slip/trip & fall hazards. • Never work or stand on a sloped or wet surface. • Work only on top of a level platform, regardless of height level. • Use the engineered devices that help you keep your balance, such as platform/ hand rails • Use proper walkway
