  1. SAP Program for Truck Drivers In order to initiate a Substance Abuse Program for truck drivers, the company must first determine if the drivers meet the SAP criteria. The SAP evaluation must be completed by a professional and should include a criminal history check, a Driver Evaluation Form, and a Medical History Form. After the evaluation is complete, the company must then decide if treatment is necessary. If treatment is necessary, the company must choose an appropriate SAP.
