  4. Waiver of Subrogation Definition of a waiver of subrogation: To waive the right of the claimant to pursue a claim against the responsible party. Purpose of a waiver of subrogation: An insurance company has the right to subrogate (or take legal action against) the third party responsible for the loss paid to the insured. When a waiver of subrogation is present in a contract, the subrogation rights of the insurance carrier are waived and the insurance carrier is unable to take legal action and recover damages against the third party responsible for the loss. Ultimately a waiver of subrogation prevents an insurance carrier from collecting payment from the responsible party after the insured has been paid.

Completed Operations Liability insurance coverage for injuries to persons or damage to property occurring after an operation is completed but attributed to that operation; does not apply to damage to the completed work itself. An operation is completed (a) when all operations under the contract have been completed or abandoned; or (b) when all operations at one project site are completed; or (c) when the portion of the work out of which the injury or damage arises has been put to its intended use by the person or organization for whom that portion of the work was done.

  7. CREATING CERTIFICATES OF INSURANCE From the AMS customer main screen 1. Hover cursor on eFormsfor a drop down menu 2. Scroll down to Certificate of Liability 3. In the sub drop down menu select new 4. In the 2nd box on the left, enter description to identify type of master cert being set up Ex: 12/13 GL, WC MASTER CERTIFICATE 5. In the box on the left titled ‘Type of Insured’ select the appropriate policies for the lines of coverage that are to be on the certificate. 6. In the 1st box on the right titled ‘Description of Operations’ type in the box CANCELLATION is 10 days for non-payment of premium.

  8. CREATING CERTIFICATES OF INSURANCE (CONT.) 7. In the last box on the right choose from drop down menu for appropriate producer name 8. In the upper right of your computer screen click on ‘Create’. Should be a dark blue line spanning the width of the screen. 9. To edit the master certificate for policy specific detail and change the producer / contact data click on the edit icon (looks like a page with a pencil laying on it ~ in the middle of the page in the blue line that spans the width of the screen). Fields that can be edited will be highlighted with a shaded background. ***BE SURE THAT THE DATA YOU SHOW ON THE CERTIFICATE IS 10. ACCURATE AND REPRESENTS THE POLICY ISSUED*** 10. When editing the master is complete, click on interests (above the Acord logo at the top of the certificate) and answer yes for saving changes.

  9. CREATING CERTIFICATES OF INSURANCE (CONT.) 11. A new screen for adding/editing certificate holders will appear. On the dark blue line that spans the width of the screen, on the far right click on ‘New’. 12. In the 1st box on the left titled ‘Name Selection’ check the button for Customer Certificate Holder List 13.In the middle of the screen on the left, where Name is highlighted red, is where the certificate holder name and full address are entered. DO NOT ISSUE CERTIFICATES WITHOUT A COMPLETE ADDRESS FOR CANCELLATION MAILING PURPOSES DO NOT SET UP A ‘SPECIMEN OR FOR BID PURPOSES ONLY’ CERTIFICATE. IF A CLIENT NEEDS ONE TO INCLUDE IN THEIR BID PACKAGE FOR JOBS ISSUE THE CERTIFICATE WITH THE CLIENT AS THE CERTIFICATE HOLDER 14. In the last box on the left click on the Write to Cust Cert Holder List 15. In the 1st box on the right side titled ‘Description of Operations’ uncheck the ‘Same as Master Description’ and that will allow you to type in the box below for detail specific to the certificate holder; i.e. job description, location description, etc

  10. CREATING A CERTIFICATE OF INSURANCE (CONT.) 16. Once completed click on ‘add’ on the dark blue line that spans the width of the screen, on the far right. You’ll see the certificate holder show up in the excel like spreadsheet on your screen. 17. To save your additions/changes to certificate holders always click on the Create/Refresh Forms button. If you don’t your information will be lost and you’ll have to re- enter it all. 18. Once you click on the button an image of the certificate with that specific certificate holder will appear on your screen… 19. To print, click on the print icon on the shaded blue line that spans the width of your screen ~ just above and to the left of the Acordlogo on the Certificate image.

  11. HOW TO RUN EXPIRATION REPORTS IN AMS 1. Click on User Tab 2. Go to Toolbox – click Reports 3. Scroll down, then click on the Expiration / Renewal Report Link, next to Customer / Policy 4. Select your Expiration Date Range. For TYPE: choose All 5. Executive, choose yourself 6. Type of business: Choose Commercial or Personal 7. Under Report Options / Sort Order: Choose Expiration Date 8. To generate report, click on magnifying glass on top right

  12. Accessing Acord Forms 1. From the AMS customer main screen 2. Go to eforms – Applications – New 3. Select General Liability and create 4. Once this acord is created you can add other acords by clicking on eforms – additional forms. 5. Choose the acord you want to create. 6. When printing, choose form overflow pages, and attachments or you will only print the page that is showing
