  2. Accident Reporting • Safety programs are designed to keep our employees safe • Accidents can still happen • Reporting accidents can help fill gaps in our safety programs • Reporting can prevent accidents from happening again in the future

  3. Accident Reporting • Slips, trips, and falls may seem insignificant to some, but may be quite hazardous to others. • All hazards should be reported so that they can be investigated and removed.

  4. Accident Reporting • Basic Safety Related Responsibilities • Taking training seriously • Knowing and following safety rules and procedures • Looking out for hazards • Reporting hazards • Reporting all accidents and injuries immediately

  5. Accident Reporting • Accidents can be opportunities • Reporting accidents can highlight problems • Once the problem is known, it can be investigated • Once investigated, the problem can be fixed • Reporting problems prevents them in the future!

  6. Accident Reporting • Not just accidents and injuries should be reported • If you notice a problem that could become a hazard, report it • Reporting hazards can prevent accidents and injuries from occurring

  7. Accident Investigation • Many things can be asked at an accident investigation. Generally, the main points covered will be: • Who • What • Where • When • Why • How

  8. Accident Reporting • Reporting accidents affects everyone • Keeping a sharp eye for hazards prevents accidents and injuries • Implementing SOP’s and safety programs help to find what does and does not work so that they may be more effective
