Case Study: • CC: right rearfoot pain • HPI: 21 yo semi-professional motorcycle jump “specialist” presents to the ED immediately after aborting a jump approximately 30-40 feet in the air. Pt landed on feet and had immediate severe pain to the . • PMH: unremarkable, denies smoking.
PE: • MSK: pain with palpation of right rearfoot including talus and calcaneus. Pain limited exam significantly. • Neuro/Vasc: DP/PT pulses 2/4, protective sensation intact. • Derm: diffuse right rearfoot ecchymosis, no blistering, blanching of skin or break down.
A/P: • Talar neck fracture: ORIF • Hawkins Classification: I (non displaced neck Fx) • Calcaneal sustentaculum tali fracture: ORIF and anterior process fracture: ORIF and Diastasis with mini-rail. • Rowes Calc Fx Classification: Type I BC.
Comments: • FDL tendon running directy over sustentaculum tali screw heads? Graft jacket for coverage? • Mini rail necessary?
