Anticipatory Care Planning at Little Cairnie Hospital, Arbroath  An anticipatory care plan is a record of the preferred actions, interventions and responses that are developed overtime, through evolving conversations, collaborative discussions and shared decision making between the person, those close to them and practitioners. Anticipatory care planning is to support those individuals living with long term and palliative conditions to plan for an expected change in health or social status. It should ensure good care planning and better access to safe and effective alternatives to avoid hospital care Wish someone would listen to what I would like to happen I would like my family member given the best care they would wish for.
