  2. NEW CLIENT ENROLLMENT

  3. Client Eligibility • Applicant must be a California resident • History of alcohol or substance abuse, and • Be an enrolled member of a Federally recognized tribe or • A descendant of a AIAN person (CDIB) • AIAN age years 12+ • or • Non-AIAN minor who is the natural or adopted child, step-child, foster child, legal ward, or orphan of a member of a Federally recognized tribe. • Non-AIAN spouse or registered domestic partner (1 year residency) of a AIAN person • A non-AIAN parent of an AIAN minor, as long as the AIAN minor meets enrollment requirements of the program

  4. Complete Client Application • Complete appropriate service assessment • Complete appropriate service plan

  5. Open Internet Explorer and type in https://kitservices3.kithost.net/CRIHBATR/ 2. Type in your User ID number and enter in the Password.

  6. Clinical Treatment Recovery Support Provider file Expectations • Application in file (including eligibility screening) • Substance Abuse Assessment • Substance Abuse Treatment Plan • Intake GPRA, hardcopy is not required to keep in file • Documentation for all services billed for • 6-month Follow-up GPRA • Discharge GPRA • Application in file (including eligibility screening) • Recovery Support Assessment • Recovery Support Service Plan • Intake GPRA, hardcopy is not required to keep in file • Documentation for all services billed for • 6-month Follow-up GPRA • Discharge GPRA
