  1. Jamestown High School Transition Program “Building skills for a more independent tomorrow.”

  3. Program • The Jamestown Public Schools Transition Program is a program within James River Special Education Cooperative. • Although housed off-campus, it is run through Jamestown High School and follows their policies and procedures. This program runs from 9:00 until 2:30, Monday through Friday, following the same calendar as the public schools.

  4. The program provides a post-secondary educational choice for students with disabilities who choose to continue working on transition skills through their Individual Education Plan (IEP) • Related services continue to be addressed according to needs of the individual

  5. Therapies continue as determined by IEP

  6. Some therapies are at the house, some are off-site.

  7. Eligibility • Must be receiving services through an IEP • Must be between the ages of 18 and 21 • Must have completed all required credits through their high school for graduation • Students participate in the graduation ceremony with their peers before attending the Transition Program • Upon completion of the Transition Program the student will receive a signed diploma

  8. Students participate in graduation activities with their friends

  9. Activities are centered around three core areas: • Vocational • Independent living • Recreation/Leisure

  10. Vocational Skills • Work experience is obtained both within the Transition House and in the community

  11. Work Sites that have welcomed us: *Humane Society *Hugos *Hi-Acres *OtterTail Power *United Presbyterian *Buffalo Mall *Hardees *Gussner Elem. *The Art Center *O’Reilly’s *Pantry Cafe *County Market *Movie Gallery *Civic Center *Ave Maria *Buffalo Mall *Alfred Dickey Library *and many others

  12. Daily Living Skills • Activities are focused toward teaching students to live more independently • Shopping • Cooking • Housekeeping • Transportation (Transit Bus) • Making and keeping appointments • Telephone Skills • Healthy lifestyles

  23. Recreation/Leisure • Focus is on teaching students to make good choices when deciding on free time activities • Board games • Dining out • Community events • Fitness • Computer activities
