  1. Customer Billing Education Package Access Service Improvement Plan

  2. Purpose • To provide customers with an understanding of charges associated with the Access Service Improvement Plan (ASIP) and how they will appear on their access bill • FCC Tariff 1 Section 5.1.1

Standard Intervals • Special Access DS1 – Address verified in Facility Availability System (FAS) • 5 Business Day Interval for On-Net (On Fiber Network) • 8 Business Day Interval for Off-Net (Off Fiber Network) • Special Access DS0 • BellSouth will confirm a six (6) business day standard interval

  4. Facility Availability System (FAS) • The objective of FAS is to provide a method for IXCs to initiate inquiries regarding the existence of fiber-fed network facilities at a customer’s premise • Prior to issuing an order for service, a customer can access the FAS via Common Access Front End (CAFÉ’) and request facility availability at a customer’s premise with an RSAG valid address • Please contact your Account Team if you need access to CAFÉ’

  5. Service Interval Determination • Access Service Requests (ASRs) must be received by 3:00 p.m. Eastern Time and be complete and accurate for the Access Order to be processed and receive an Application Date of that day’s date • Service intervals are calculated with day 1 being the first business day after the Application Date • Service dates are communicated to the customer via the Firm Order Confirmation (FOC)

  6. Changes to Pending Orders • For services with standard intervals where the service interval between the subsequent request date and the subsequent due date is four business days or less (if the request is received before a Customer miss on DD/SD), a service date advancement charge will apply for each day the new service interval is less than the standard interval.

  7. Changes to Pending Orders • For services with standard intervals where the DD/SD has a Customer miss, and the service interval between the subsequent request date and the subsequent rescheduled due date is two business days or less, a service date advancement charge will apply for each day the new service interval is less than the rescheduled standard interval of 3 days. This provides a cap on advancement charges for rescheduled appointments due to a Customer miss of $400.00 per circuit per order.

  8. Missed Due Dates by BellSouth • Irrespective of whether the company meets the agreed upon service date for an access order but does provide the service prior to the standard interval for the service ordered, a service advancement charge will apply for each day the company completes the access order in advance of the standard or negotiated interval. • Note: Early completions will not increase the service date advancement charges.

  9. Service Date Advancement Charges • $200 per day per circuit Service Date Advancement Charge – USOC SDASP • This charge is automatic if a customer requests an earlier due date than 5 business days for On-Net DS1, 8 business days for Off-Net DS1 or 6 business days for DS0 • The charge applies at a rate of $200 per circuit for each day the circuit is advanced from the standard or negotiated interval • The number of per day charges will equal the standard interval minus the number of days in within which the service was installed. There is a 7 day cap on service date advancement charges for initial requests.

If multiple circuits are involved, the SDASP will be billed based on the number of circuits, but the other circuits won’t be displayed for the charge.) This circuit was a non-fiber fed location with an 8 business day standard interval. The service was installed in 2 business days. Service Date was advanced 6 business days. ONE-TIME CHARGE FORService Date Advancement SDASP 6 SERVICE DATE ADVANCEMENT CHARGE, SPECIAL charges are $200 for each ACCESS SVC AND CLECS, PER CIRCUIT, PER DAY day theinterval was PIU 100 improved. Calculations = INTERSTATE - FL - EC 5191 1,200.00 $ 200 per day X 6 days = $1,200 Service Advancement Charges

Additional Dispatch Charges • $150 Service Date Change/Additional Dispatch Charge – USOC - OMCAD • This charge will apply when a BellSouth technician has been dispatched to the customer’s premise to install, change, or rearrange a service for the customer and for whatever reason, the customer is not ready for service and did not notify the company in advance. This charge applies for each occurrence of a technician dispatch to a customer’s location. • $31.60 Service Date Change – USOC – OMC • This charges applies on a per order, per occurrence basis for each service date changed.

  13. What Your Bill Will Look Like Additional Dispatch Charges This PON is for a customer that was not ready on the confirmed due date. A BellSouth tech was dispatched to the end user’s location and was not able to complete the turn-up due to customer not ready. The PON was supped for a new due date and the customer was charged for a service date change charge and an additional dispatch charge. ADDITIONAL DISPATCH CHARGE PIU 100 INTERSTATE - FL - EC 5191 150.00

  14. Customer Missed Appointments on Advanced Service Requests • $300 per circuit per occurrence of a missed appointment situation for customer reasons • 3 Day Minimum to reschedule missed appointments for customer reasons without additional service date advancement charges – capped at $400 per circuit

  15. Discounts • A 20% Discount will be provided for orders with intervals greater than or equal to 16 days • The 20% Discount applies to Special Access DS1 Service Local Channels, Interoffice Channels for installations, moves or rearrangement of service • 20% Discount does not apply to Project Managed Orders

  17. Requirements for 1- 4 Day Intervals • Customer’s must provide access to premises until 9:00 p.m. Local Time and not require coordinated testing for basic BellSouth DS1 with requested service intervals less than 5 business days. Customers will receive a note on the FOC – “9PM ACCESS REQUIRED” for these service intervals

  18. Requirements for 0 Day Intervals • Same Day or “Zero” Day Intervals will only be available for Telecommunications Service Priority (TSP) emergencies and will not be available for other Access Service Requests (ASRs).

Frequently Asked Questions • Where can I find ASIP information in the tariff? • FCC Tariff 1, Section 5.1.1. • How late must I provide access on an order with an interval of 4 days or less? • Access to the premises must be permitted until 9:00 p.m. local time on all 1 – 4 day interval orders. • Why am I being billed $31.60 and $150 for changing the due date on an order? • Any change in the service date results in a $31.60 charge and if the due date change results in a subsequent dispatch to the customer’s premises, an additional charge of $150 will be applied.
