From your Home Page, click on “Administration” and select “Add Reporter” • Click on the binoculars icon • When the “Find a user” box opens, enter the new reporter’s netID in “Verify a User” • Click “Find” • Select the Reporter’s department • Check “PHS/42 CFR 50 Agency” if the reporter is PHS funded • Click on “Create Reporter” to add this profile to the database. Search for Reports • From your Home page, Click on “View Reports” and then on “Search COI Reports”. • Search: • For a reporter: Enter the reporter’s netIDand go to Step 3. • For reporters in certain statuses: select a status from “Report Status” • For reporters in certain colleges: select a “College” • Click on “Find (List)”, or simply push “Enter” on your keyboard. • Search results will look like this. Click on the name to see the details of the report and get to the “Edit Reporter” screen to make edits to the reporter’s profile. 1 2A 3 2B 2C 4

Search for Non-Compliant Reporters • From your Home page, Click on “View Reports” and then on “Search COI Reports”. • In the “Report Status” box, hold down Ctrl + Alt and select “In Work” and “Un-started” • Click on “Find (List)”, or simply push “Enter” on your keyboard. • Search results will look like this. Click on the name to see the details of the report and get to the “Edit Reporter” screen to make edits to the reporter’s profile. 1 2A 3 2B 2C 4

Click this to: Start the Annual Report, OR Submit Travel or Award Specific Report (PHS funded reporters only), OR Report a change in financial interest over the course of the year Next slide

Three most recent reported Award Specific reports are shown here. - Award specific reports serves as disclosure from the reporter if they have an external financial interest that is related to the activities of that award. Three most recent reported reimbursed or sponsored Travel are shown here.

  13. Non-PHS funded Summary Page • This is what your Summary Page will look like if you are not funded by a PHS agency.   • You will see all the entities that you have previously reported. If there is an entity listed that you no longer have a relationship with, you may deactivate it here. If this is your first report or if you have not reported external interests, this section will be blank. • **We strongly encourage you NOT to “Add an Entity” before going through the Screening Questions.* * • Proceed to your Annual Report by clicking on “Continue”. • If you are on this page because you have forgotten to add an Entity, click on “Add an Entity” and then Click “Continue” to proceed to the Screening questions. 1 2 3

PHS funded Summary Page • This is what your Summary Page will look like if you AREfunded by a PHS agency. • You will see all the entities , travel and award reports that you have previously reported. If there is an entity listed that you no longer have a relationship with, you may deactivate it here. • **We strongly encourage you NOT to “Add an Entity” before going through the Screening Questions.* * • Step 1: If you have an active award from a PHS agency which does not appear in your list, add it by clicking on “Add a report.” • Step 2: If you have undertaken any reportable travel within the past 12 months which is not reflected in your list, add it by clicking on “Add New Travel.” • Step 3: Add a new entity by clicking on “Add a New Entity” only if you are certain that it is one which should be reported. Otherwise, feel free to skip this step for now. • Step 4:Click on “Continue” to proceed to the Screening Questions in your Annual Report. Step 1 Click on the Information links to learn more about whether you should report any award/travel or financial interest. Step 2 If there is an award or entity listed that you no longer have a relationship with, you may deactivate them here. There is no need to deactivate previous travel. Step 3 Step 4

Entity List Page • You may realize that you have duplicates or have missed reporting an entity. • Follow the Steps 1, 2 and 3 to navigate through this page. • If you need to add or remove an entity, click on the link provided to go to the “Summary” tab where you can make the changes you need to. • If you make changes to any information on the “Summary” tab, you will be taken through the Screening questions again, so that you can review and update the answers to the questions accordingly. All the entities previously entered are displayed here. If you have multiple entities in this list, you will come back to this page, when you complete a detailed report for each entity. The “Next” button will not be available until all the entities are in “Completed” status. After you complete an Entity report, its status will show as “Completed”. You can “Reopen Report” if you want to make any changes. Otherwise click “Next”.
