What is the Role of Diabetes Community Health Workers? • CHWs are individuals who represent their ethnic, cultural, or geographic communities and provide a link between these communities and health care providers. • CHWs are well positioned to engage the people in their community to prevent diabetes and its complications through education, lifestyle change, self-management and social support.
