  4. Clinical Governance Definition

  5. Clinical Governance • A framework/system for continuously monitoring and improving the quality of care the NHS and its services provide • Safeguarding high standards http://www.clinicalgovernance.scot.nhs.uk/section1/definition.asp

  6. Continuing Professional Development

  7. The Ten Principles of CPD • Principle 1, 2 & 10 set out the important relationship between appraisal, CPD & revalidation: • Principle 1 includes a PDP outside a narrower speciality interests • Principles 2 is a balance of learning methods that includes a component of active learning • Principle 10 is about the consequences of failing to provide evidence or insufficient evidence • Support Good Medical Practice • Professor Nigel Sparrow's paper ‘Good CPD for GPs’ describes the principles • http://www.rcgp.org.uk/PDF/ACADEMY%20GUIDANCE%20CPD%20HEADINGS.pdf

  8. CPD aims: • Continuing the learning process • Maintaining & improving our standards across all areas of practice • Supporting change to practice & career development • Helping keeping up to date even when not practicing • Professional Responsibility

  9. Professional Development Board • Meet 3 times a year • Direct educational & professional development • Effective delivery of a comprehensive CPD Scheme • For all UK and overseas members of RCGP

  10. CPD Credit Scheme • A minimum of 50 credits is required (per annum) • A broad range of general practice should be covered in 250 credits over 5 years • Credits will be self-assessed and verified at Appraisal • The scheme is intended to fulfil the standards and requirements of CPD for all GPs for Revalidation

  11. CPD Credit Scheme • One hour of learning (including planning & reflection) equals one credit • You may get a certificate of completion or attendance but it the is the self assessment and applying it to work that counts • Reflection • Acquisition of knowledge/learning points - one hour (1 point), relevance to his/her working situation • Planning e.g. case study demonstrating change in practice, reflective practice or audit of patients one hour ( 1 point) • Demonstrating impact (multiplies by 2)

  12. Impact of CPD?? • Patients • (e.g. a change in practice, implementing a new clinical guideline, initiating a new drug for the first time) • The individual • (personal development, e.g. development of a new skill or further development of existing skills) • Service • (e.g. developing and implementing a new service, becoming a training practice, teaching others) • Others • (teaching, training, NHS locally or nationally).

  13. Appraisal

  14. Appraisal • To enable you to continually improve the quality of your practice • Early identification of doctors in difficulty • Aim to meet the UK regulatory requirements for revalidation • In appraisal – demonstrate evidence of CPD • Strategy Paper ‘Principles of GP Appraisal’ • Endorsed 28th Feb 2008

  15. Appraisal • PCT responsibility • Local Appraisers • Develop & maintain PDP - central to Appraisal • Personal Development Plan (PDP)

  16. Revalidation Aims to assess whether GPs meet current professional standards & are fit to practise

  17. Revalidation • 5 year Revalidation Cycle • Encompasses both: • Relicensure - licensed doctors practise in accordance with GMC generic standards • Recertification – meeting the standards of the GMC’s GP register

  18. RCGP Revalidation Eportfolio • A new appraisal & revalidation toolkit • A transitional & lifelong tool • Released in phases –first the ‘Appraisal’ Phase • Supports GPs, GP appraisers & primary care organisations (PCOs) • Traffic Light System

  19. 12 Areas of the Eportfolio • Area 1: Statement of Professional Roles • Area 2: Exceptional Circumstances • Area 3: Appraisal form 45 • Area 4: PDP • Area 5: PDP review • Area 6: CPD credits • Area 7: Colleague Feedback (MSF) • Area 8: Patient Feedback (PSQ) • Area 9: Complaints/Cause for concern • Area 10: Significant Event Audit • Area 11: Clinical Audits • Area 12: Declarations

  20. CPD Entries • Areas • Clinical • Professional & Academic • Managerial • Dimensions • Personal • Local • External

  21. Summary • A brief glance of a comprehensive professional & educational development structure • Overall the aim is to have standards of excellence for the NHS and its services, which we as GPs need to meet to provide the best care for our patients • We have to provide evidence which is being integrated into the eporfolio • The eportfolio will support annual appraisals and 5 yearly Revalidation • Both appraisal & revalidation can help support widening our knowledge and career development plus help identify those in difficulty
