Kilburn Park Medical Centre Recovery and Wellbeing College in Practice • PROJECT AIM • To replicate the CNWL Recovery & Wellbeing College model within a General Practice setting. • To assess whether by learning together in a bespoke Healthy Living Programme, patients develop enough understanding of conditions, enough confidence in managing their health independently and enough resilience, to have a measurable impact on their subsequent patterns of contact with a clinician. • Evidence suggests that the top 3% of GP attenders require around 10% of the primary care resources.

Kilburn Park Medical Centre WHAT WE DID... • Held focus group meetings with a group of patients • Together, decided on a programme of courses • Together, decided on criteria for enrolment • Promoted college idea within the practice and met with staff and patients • Advertised college courses to all staff and patients • Students could be referred or self refer • Organised Learning Plan Sessions with a Learning Advisor • To evaluate project outcomes; selected two PROMS (Patient Reported Outcome Measures) • Collected baseline data on wellbeing as students enrolled with the college • Located local venues where courses could be held • Co-ordinated courses, venue availability and trainers from CNWL Recovery College • Ran 18 half-day workshops, 1 six week course, 1 five week course, 1 four week course and 1 two week course
Kilburn Park Medical Centre • Over the 5 months - 120 unique students enrolled with the college. • 22 courses/workshops took place between 29th April and 30th September 2016 (some of these were multiple week courses) • 327 attendances have been recorded • Health demographics • Almost 80% of the students had anxiety or depression • Over a third had recorded that they had experienced some form of violence or abuse (39%) • Over half suffered from some form of chronic pain (52%) • 11% had recorded problems with substance abuse, mostly alcohol dependency • 7% had communication difficulties; literacy, autism, or mild learning disability • Almost 30% had a BMI of >30 (obesity) • 68% of patients who enrolled were amongst those previously attending the GP more frequently • 78% of cohort live alone
Kilburn Park Medical Centre • Baseline clinical contacts at GP (list 7,800) prior to intervention: • > 500 patients had > 20 clinical contacts/15 months (range 20-93) • > 1800 patients had > 10 clinical contacts/15 months • Reduction in clinical contacts after intervention in College cohort: • Overall average reduction of clinical contacts: • 2.2 per patient • Reduction in clinical contacts in patients who had previously had more clinical contact: • 9.25 per patient
