CAKE Session 17 Compliance - Work Permit
Purpose of the Work permit system: • The Purpose of the system is to: - Prevent undesired incidents and mitigate elements of risk. - Document safety measures taken - Communicate information about work activites to personnel that are or may be influenced by the work. - Handle and control interfaces between activities on the installation.
- Work Permit: • A Work Permit (WP) is: • A specific permission • to certain personnel • to perform a defined work • in a defined area/place • at a defined space of time • within certain circumstances • to coordinatejobs • An operational barrier against undesirable incidents
High risk activities • WP Level 1– potential HIGH RISK activities • Need for clearance and coordination on installation level • Hot work class A • Hot work class B in classified area • Entry • Disconnection of safety systems • Work on Hydrocarbon- carrying systems • Pressure testing • Work over sea • Work involving hazardous chemicals, radioactive substances, and low specific activity scale (LSA) • Well operations/well interventions • Work involving explosives • Critical lifting operations
Medium risk activities • Level 2– Other activities that due to risks • requires coordination and clearance within a area or a system • Mechanical work • Work on electrical equipment • Work on automation-, data and telecommunication systems (not including safety systems!) • Scaffolding • Routine work on hydrocarbon-carrying systems where there is no risk of hydrocarbons being released • Work on chemical systems where there is no risk of chemicals being released • Isolation work • Hot work class B in unclassified area • Paint work
Work that does not require WP: • There are several activities that may be carried out without a WP. In general this applies to normal routine operations within: • Production • Drilling • Logistics • Marine • Activities in workshops, approved for the specified work • General work inside living quarters • Inspections performed without physical intervention • Cleaning and tidying that does not involve use of WP chemicals
SJA and Toolbox Talk • Involved personnel should constantly consider the need for: • Safe Job Analysis for WP 1 and WP 2 • Are all risk factors identified an handled by the WP? • Known difficulties or former incidents? • Are the task described in a procedure or checklist? • Crew’s experience? • Known equipment? • And remember to follow the recipe!
Toolbox Talk: • Important to remember: A Toolbox Talk shall always be conducted before starting your work!
Area Responsible • AR is overall responsible for the planning and coordination of all activities within the area. • Shall review all WP in his area before approval • Responsible for activation and deactivation of WP, but: • This role may be delegated to all personnel under his responsibility (as Area Technician), if written. • AR System: Owner of the electric, pneumatic or hydraulic system affected • AR Area: Owner of the physical area affected • AR Department: Responsible for the personnel performing the work Note: There might be several Area Responsible persons involved in one WP!
Area Technician • The Area Technician that has been given the authority by it’s AR, in writing. • Responsible for the pre-job check of the work site • Prior to activating the WP! • Ensure that all safety precautions are in place • Ensure safety systems are handled or isolated according to arrangements, including barriers • Responsible for the post-job check of the work site • Upon completion and deactivation of WP • The work site reset to normal, including all safety systems and barriers. • The work site shall be visited physically! • Work Permit Administrator to be notified
Duration of WP 12:00hrs / 7 days • WP level 1: • A WP level 1 is valid for up to 12 hrs and expires at the end of the day or night skift. A new WP must be issued if a shift ends without having completed the job and a new shift has to take over. • If the job lasts for more than 12 hours tha Area Responsible can extend the duration for up to 4 hrs given the job can be finished with the sam number of personnel. A new WP must be issued if a new shift has to take over. • WP level 2: • A WP level 2 is valid up to 7 shifts (12 hrs) for up to 7 days . The WP has to be cleared again if a shift ends without having completed the job and a new shift has to take over. • If a job lasts for more than 12 hrs, the WP level 2 has to be cleared again with the new Area Technician.
Cancellation of WP: • A WP can be cancelled or held back at any time, depending on the situation on the installation. Work must be stopped immediatley and the work site must be secured. • In the event of emergency alarm or production shutdown • In the event that the preconditions for the WP are breached. • In the event that a dangerous situation develops, or if operational precautions so dictate • The WP is invalid if: • The WP requires gas measurement and the work is not started within one hour after the gas measurement has been carried out • The scope of the work has changed, and no longer corresponds with the descriptions in the WP.
MÆRSK DRILLING Only the OIM and the Lead Driller (at night) can approve WP Level 1 - Excluding: Hot work class A Safety Systems • Excemption for • MÆRSK INSPIRER for StatoilHydro • Only the OIM can approve WP Level 1 - including: • Hot work class A • Safety Systems
