I have no knowledge of any physical impairment that would be affected by the above athlete’s participation in the program as outlined above. I give my permission for my child to be treated by a physician and/or hospital should immediate attention be deemed necessary and recognize that any fees incurred will be my sole responsibility. I hereby waive and release Char Wilson, SHGFS and ASA of any liability or illness sustained while participating in Training Classes. I also understand that Back to Basics retains the right to use photographs of athletes taken during the class for publicity and advertising purposes. 24Hr Cancellation Policy: if we are not notified 24 hours prior to your scheduled session you will forfeit any payment for that class session. You will be required to pay for your missed session prior to beginning a new session. No refunds will be given for missed sessions. Back to Basics and Char Wilson reserve the right to cancel or postpone the class at any time and without notice. Should Back to Basics be cancelled by Char Wilson, all monies will be refunded.
