  4. CAPF 104 - Header • Top of form contains mission number, date, sortie number. • Below that, crew and aircraft information. • Write in Names of Crew

  5. CAPF 104 – Flight Plan • Right side contains flight plan. • If not on a FAA flight plan, Flight Release officer is responsible for ensuring the aircraft arrives at it’s destination. • Similar information to FAA flight plan form.

  6. CAPF 104 – Search Plan • Left side contains search plan. • Mission Objectives • How to search • Where to search • Weather • Communications plan • Hazards • What to do if found.

  7. CAPF 104 - Signatures • Bottom is the signature block. • Should be signed by pilot, briefing officer and the dispatching or flight release officer. • Briefing officer and Dispatch signature may be the same person, but not necessarily. • This is the signature for the flight release. No separate CAPF 99 required, but should be logged on CAPF 107.

  8. CAPF 104 – Briefing Notes • Always fill out the form completely. • Front side should be complete before departure. • If something doesn’t apply, put N/A. • This is the flight release for the flight and is legal documentation of what was planned for that flight. • Keep a copy at base.

  9. CAPF 104 – Search data • Data about the area actually searched, search conditions and crew effectiveness.

  10. CAPF 104 - Notes • Have crew add notes. • If attaching logs, put see attached logs. • Draw maps of anything unusual that might even be closely related (no artistry required).

  11. CAPF 104 – POD • POD calculations. • Circle appropriate number or write off to the side somewhere.

  12. CAPF 104 – Debriefing Notes • Always fill out the form completely. • Back side should be completed ASAP after arrival back at base. • If something doesn’t apply, put N/A. • This is one of the most overlooked items on a search. • Be thorough. Planning will use this information to help decide where to search next. • This is the legal documentation of what happened on that flight.
