Student is referred by teacher: SST chair adds student to system
SST Chair adds student information: Once teacher is assigned, this will give he or she access to log in and complete forms. Teacher
Teacher logs in and sees home page: Click on the “forms” icon to take you to the Forms Page.
Before SST: Teacher completes 2 to 3 forms Teacher (ELL form if applicable) SST chair completed general info when we added student.
Teacher completes: Pre-SST Interventions Save, print, data tracking. Mark status of form and add comments.
Teacher completes: Student Referral (cont.) ELL student? Complete Supplemental ELL form.
Teacher completes: Student Referral (cont.) Student data from multiple years can be added.
Troubleshooting and Technical Support On the homepage click on the “support” link for help desk information.
Update password to something secure • Enter email to ensure it is correct • This is important because the system uses email addresses for password retrieval and to let teachers know about important SST events Remember

Only the interventions attempted are checked off. Once the form is completed the green check mark is clicked at the top of the form.
End of Pre SST form. Teacher indicates outcomes and how long interventions have been in place.
After form is completed and saved the teacher clicks the green checkmark at the top of the page.
Even though Math and Written Expression are not areas of concern the present levels are indicated.
Areas that aren’t a concern are indicated. Since Jane is identified as ELL, the teacher must now complete the ELL Supplemental Form
Continuation of the Student Referral Form. Universal screening data should be included, if available.
Completed since Jane is an ELL. Note: Only the items which the teacher knows are completed. Others are left blank. The form can be edited later. If you’re unsure then do not guess. Interviews with the parent can be conducted during the SST meeting to confirm answers that are relevant to the problem.
