YOUR BENEFITS | core benefit solutions that work for you NON-SMOKER MEDICAL PREMIUM DISCOUNTS Tobacco use continues to be the leading cause of preventable death in the U.S. and can lead to serious illness, disability and decreased quality of life. Floor and Decor is committed to managing/minimizing healthcare cost increases for employees and the company. For those members who are committed to a healthier lifestyle by not smoking or using tobacco, Floor and Decor has implemented non-smoker medical premium discounts. • Premiums will be approximately 25% lower for those members who do not smoke or use tobacco. • Each member will be asked to verify his/her smoker status when you call to enroll.

Keep in mind, if your doctor charges more than the Plan’s “reasonable and customary” charge, you may be required to pay the extra amount.

A key benefit to this plan is that every 12 months you can get an exam and lenses (or contact lenses).   12. YOUR BENEFITS| Basic Life and AD&D Insurance Floor and Decor provides all eligible employees with a basic life benefit at no cost to you. Benefit Amounts: For You: 1 X your annual salary to $500,000 max This benefit is paid for by Floor and Decor for you, our valued employee

VOLUNTARY LIFE BENEFITS | there when you need it If you want additional protection for you and your family, you can buy: *This benefit has a one time open enrollment that allows employees to increase their 2011 voluntary life insurance benefit coverage by $50,000 without submitting a health application. If you decline coverage at this time and wish to add it at the next enrollment period, you must complete health questions and be approved for any amount of coverage.

  14. VOLUNTARY SHORT TERM DISABILITY | there when you need it Protecting your paycheck is important. You insure your house, your car, your teeth, and your health but if you don’t have a paycheck then all those benefits go away. The definition of disability is your inability to perform your own occupation.

  15. LONG TERM DISABILITY | there when you need it Protecting your paycheck is important. You insure your house, your car, your teeth, and your health but if you don’t have a paycheck then all those benefits go away. Floor and Decor pays 100% of the cost for Long Term Disability.

  16. ImportantNote! FINANCIAL BENEFITS | Flexible Spending Accounts Flexible Spending Accounts Spending Accounts work like a savings account. Each pay period a pre-tax payroll deduction is deposited into your Health Care and/or Dependent Care Spending Account. Unclaimed balances left in your FSA at the end of the plan year are forfeited and not returned to you. Please give careful thought to the amount you want withheld. Be conservative in your estimated annual expenses. Contributions are automatically deducted from your paycheck.

HOW TO ENROLL | Benefits Enrollment • Call Willis at 800-874-2244 and speak with a benefits enrollment representative. • The representative will guide you through the enrollment process by asking for your election decisions. • Your decisions will then be updated in the online enrollment tool by the representative. Dependent & Beneficiary Updates: Prior to enrolling, go to the employee access website, https://portal.adp.com ,and update dependent and beneficiary information.
